For Referring Physicians
Welcome to Southern Vascular & Interventional Pain Management! If you’re new to our practice, we’re delighted to have you. If you’re one of our valued referring physicians returning to us, thank you for your continued trust.
At Southern VIP, our mission is to collaborate with referring physicians to achieve the best possible long-term outcomes for patients. By utilizing cutting-edge interventional technology and techniques, we aim to go beyond merely alleviating symptoms to finding lasting solutions that enhance patients’ quality of life.
Our referring doctors rely on us because of our streamlined referral process, consistent communication, and outstanding patient outcomes. We take pride in hearing from patients who report improved well-being, reduced medication needs, and a readiness to be more active. These positive results are a shared success for all of us.
Referring to Southern VIP
We want referring patients to Southern VIP for vascular care or pain management to be easy. We ask for one simplified referral form (which can be printed on this page) as well as supporting demographics, insurance info and medical history. When completed, fax over the information to the office where you would like the patient to be seen. That’s it. You’re done.
- Memphis office fax (901) 350-0677
- Union City office fax (731) 507-0273
For all PAD and Vascular patients fax 901-350-0677
Then we take it from there. We will follow up with insurance verification, discuss-out-of-pocket expenses with the patient upfront so they have no surprises, and then quickly get them scheduled. Most patients are seen within a week to 10 days.

Foot Rescue is a minimally-invasive outpatient procedure for PAD patients performed by Dr. Hodgkiss, or another specially trained IR, at our new Southern VIP surgical procedure facility in Fulton, Kentucky. Southern VIP is dedicated to eliminating unnecessary amputations due to Peripheral Arterial Disease that is literally cutting lives short by proactively addressing blockages using minimally-invasive Foot Rescue procedures.
Most vascular surgeons and interventional radiologists only address blockages in the leg, and that is what makes Foot Rescue different. The IRs at Southern VIP are uniquely trained for below-the-knee (BTK) and pedal loop revascularization, including the most difficult cases where others may have recommended amputation. Stop cutting and start treating with Foot Rescue. See pedal loop revascularization example below.
